This Request for Proposals (the “RFP”) is an invitation by the Town of Cochrane (“the Town”) to prospective proponents to submit proposals for Community Wellness Plan, as further described in Section A of the RFP Particulars (Appendix B) (the “Deliverables”).

The Town is seeking the services of a qualified consultant to provide expertise to research and develop a Community Wellness Plan (“Plan”). This Plan will focus on community-based prevention and intervention strategies and initiatives to tackle gun and gang activities within the community with a focus on youth. Once the Plan is complete the consultant will be responsible for the implementation and sustainability of the Plan for the first year.

The Town’s Community Wellness Plan is to be completed on or before March 31, 2025. First year of implementation services must be completed by March 31, 2026. Final reports are due 90 days after the March 31, 2026, deadline.

The Term of the Agreement is to be for a period of three (3) years, with an option in favor of the Town to extend the agreement on the same terms and conditions for an additional term of up to 2 (2) years in one (1) year increments at the Town’s sole discretion.
